8D analysis is a structured eight-step problem-solving method, widely used in industry to address failures and improve processes. Here are the details of each step:
- Team composition (D1):
- Form a multidisciplinary team with varied skills to guarantee a complete and diverse approach to the problem. The team must include members with specific knowledge of the process or product concerned.
- Problem definition (D2):
- Use the 5W2H tool (What, Who, Where, When, How, How much, Why) to define the problem precisely. This step ensures that everyone fully understands the nature and extent of the problem to be solved.
- Implementation of containment measures (D3):
- Put temporary actions in place to protect the customer or the process from the effects of the problem. These measures aim to limit the impact of the problem until a definitive solution is found.
- Identification and verification of causes (D4):
- Identify the root causes of the problem using tools such as the 5 Whys or 7M analysis (Machines, Management, Environment, Measurements, Manpower, Equipment, Materials and Methods). Verify the causes to ensure they genuinely explain the problem observed.
- Determination of corrective actions (D5):
- Develop and select permanent corrective actions to eliminate the root causes. These actions must be tested and validated to guarantee their effectiveness before being implemented.
- Implementation of corrective actions (D6):
- Apply the selected corrective actions and monitor their implementation. Ensure clear communication and adequate training where necessary so that all stakeholders understand and apply the changes.
- Prevention of recurrence (D7):
- Modify systems, processes, procedures or designs to prevent the problem from recurring. This step may include updating reference documents, revising standards and improving quality controls.
- Closure and recognition (D8):
- Close the process by documenting all the steps of the problem resolution and evaluating the results obtained. Recognise and celebrate the team's work, and share the lessons learnt to strengthen the culture of continuous improvement within the organisation.
8D analysis structures problem solving methodically, guaranteeing precise identification of causes and effective implementation of solutions. It is supported by a document (on paper or as a computer file) that enables actions to be tracked and traced until the problem is fully resolved.
